Notes:

Final confirmation of the number of meals needed (please include any volunteers eating) must be made by the Monday prior to the event. This will represent the minimum you will be charged for. Additional meals may be ordered, dependent on availability.
Final players list and golfer count due at the Pro Shop two (2) days prior to event. At least one name required from each team (the more names provided, the better).
